APPLICATION FOR MEMBERSHIP

I desire to become a patron / Life Member / Member of the Kota Cancer Society, Kota hereby apply for enrolment as a member.

I do hereby agree that in the event of my enrolment, I will conform to the Articles of Association; By-laws (Rules and Regulations) of the society as are in force or as may be altered, amended or enlarged so far as I am affected thereby.

I also agree to pay the donation / subscription as prescribed in the By-laws. I further agree to promote the objects and aims of this society as far as shall be in my power and attend the meetings of the society as often as I conveniently can.

I undertake that in the event of my desire to resign from my membership of the society. I agree that I shall first pay the dues of the society, if any due from me.
